To companion the end-of-life journey with skill and compassion.

Central Wyoming Hospice & Transitions Program (CWHTP) is a 501(c)3 nonprofit organization that exists as the premiere provider of end-of-life care for individuals and their families and caregivers. Central Wyoming Hospice and Transitions has been providing services to patients and families since 1981, and currently serves more than 400 patients a year. We exist to provide hospice and Transitions services for individuals and their families, as well as grief care for the community as a whole.

Our Staff and Board

Our team of qualified, caring professionals exist to be a hand to hold during life’s most difficult moments. Our staff is highly trained to be a physical, emotional, and spiritual support system for patients and their families.

Kilty Brown, MPA, RN

Executive Director

After graduating from the University of Texas Health Science Center School of Nursing, Kilty moved to Wyoming to embark on a career in healthcare. As a nurse, she worked across the continuum of healthcare until she found her passion in hospice and palliative care. She considers providing comfort in an individual’s last days one of the greatest honors of her career.

Kilty is active in the palliative care community both regionally and nationally. She gives back to the community through Meals on Wheels and the Casper College School of Nursing Advisory Board.

The Central Wyoming Hospice & Transitions Team is equipped to provide the support and care you need.

  • Our Clinical Team has over 170 years of collective experience in providing hospice care.

  • We value education. All clinical staff receives specialized training in end-of-life care. Our nurses and aides continue this commitment with ongoing clinical education and working toward Hospice and Palliative Care certifications.

  • Our staff is available 24 hours a day to meet your needs. We are here to help you stay at home and not return to the hospital.

  • We have a strong core of more than 80 trained volunteers. Our volunteers cook meals, provide companionship, sit vigil, and so much more.

  • We are a non-profit organization. That means we only answer to our patients, their families, and our community.
