Transitions Program
When it comes to personal care, there are so many different aspects to consider. Medical needs, prescriptions, appointments, endless amounts of paperwork, and more prove that it’s easy for our loved ones to get lost in the logistics. Sometimes they need a doctor to tell them what medications to take. Sometimes they need a lawyer to tell them what forms to sign.
But sometimes, just sometimes, all they need is a friend. We are here to be that friend.
Since 2005, our Transitions program has helped our community members stay as independent as possible by offering extra support and connections to community resources. Transitions is a non-medical community service program that utilizes volunteers to provide assistance, companionship, and other services to those who need a helping hand.


Transitions services are provided by our Transitions Coordinators and Volunteers. Each client has a Case Manager to help match their needs to available volunteers.
Services include:

Visitation and companionship

Assistance with writing and reading correspondence

Transportation and running errands

Respite breaks for families and caregivers

Referrals to community resources

And more
The Transitions program is provided at no cost, and you don’t need to be on hospice to utilize this service.
