Hospice Care

When your loved one is entering their final days, it is a whirlwind of emotions for all parties involved. There is fear, love, sadness, anger, and so many other emotions that it can become overwhelming trying to figure out the next step. Central Wyoming Hospice & Transitions exists to help guide you through those next steps with care and compassion.

Since 1981, Central Wyoming Hospice & Transitions has been providing end-of-life care, but we are more than just the end and our hospice services show that.

We are there for you through it all. From the happy, to the hard, and everything in between - we are with you.

We offer care, including symptom management focused on keeping your loved one comfortable. Our clinical staff has over 170 years of collective experience in providing hospice care and will work with you and your family member to ensure those final days are full of as many happy, peaceful, comfortable moments as possible.

We also recognize that your loved one is so much more than just their medical condition, which is why our hospice care services utilize an entire team, including social workers, a chaplain, a grief care expert, and various volunteers who will provide personalized care to help your loved one through their final days with the peace, compassion, and dignity that they deserve.

We know that home is where the heart is and we try to bring that sense of home to each and every one of our patients. We offer short-term and long-term stays in our own Hospice Home facility, but can also provide in-home nursing care wherever our patients reside. Wherever they call home, we will meet them where they’re at.

Whether they are in an assisted living facility, a nursing home, their own home, or our own hospice home, your loved ones will know that they are safe and taken care of; physically, mentally, emotionally, and spiritually.

These moments are hard for everybody, but you’re not in this alone. We are here to support you and your loved ones.

Transitions

Transitions is a free program available for seriously ill individuals, who may still be seeking curative treatment. Transitions offers you a case manager to help connect you to resources in the community. Additionally, volunteers offer companionship, can run errands, offer meal preparation, provide short respite breaks, and more. Our goal is to provide you that extra support to keep you as independent as possible.

Grief Care

Central Wyoming Hospice & Transitions offers a grief support system. Our grief services are available to the general public as well as our hospice families at no charge.

Facing end-of-life issues or rebuilding a life after death can be one of the hardest things with which an individual, family or loved one must contend. We are here to help with that part of the journey as well. We understand that grief comes in many forms and is unique to every individual. It is also a normal and a natural response to loss. It can affect a person’s daily activities, routines, relationships and the way someone lives.
