Caring Transitions and Assistance
Transitions is a free, non-medical community service program for individuals with serious illnesses who may still seek curative treatment. The program connects clients with case managers and volunteers who provide:
- Companionship and visitation
- Help with correspondence
- Transportation and errands
- Respite for caregivers
- Referrals to community resources
Since 2005, Transitions has helped community members remain independent by offering personalized support and connecting them to essential resources. Each client is paired with a case manager who matches their needs with available volunteers.
This program is available at no cost and does not require hospice enrollment.
