Hospice Registered Nurse

Job Summary: Provides nursing care to individuals and families based on nursing appraisal and provides case management and coordination of assigned patients.

Hospice Aide

Job Summary: Provide personal care services to Hospice patients under the general supervision of a Registered Nurse. Assist patients and families in maintaining a safe and healthy environment.

Chaplain

Job Summary: Work to affirm the spiritual dimensions of life and death and provide patients and families with the spiritual comfort and support needed.

Nurse Practitioner

Job Summary: Provides quality palliative care to assigned patients in consultation with the hospice Medical Director. Will work in collaboration with the Interdisciplinary Team (IDT), patient, and family provides consultation, assessments and evaluation of the plan of care.
